Now more than ever, it’s imperative for hospitals and other health care providers to have a strong game plan for caring for patient populations. The rise of accountable care offers financial incentives for physicians and hospitals to improve population health. Notably, one of the main premises of an accountable care organization is to care for a set population and ensure that a group of patients are guaranteed care across a continuum.
Please join us for an inspiring discussion on Population Health Management and the relationship between the employer and the health plan. Jan Blue, Chief Human Resource Officer from Hoag Health and Dr. Jeffrey Klein, Senior Medical Director for Cigna will speak from the employer and clinical management perspective as to the value of a strong population health management strategy and the benefits on medical risk and cost management, improved health outcomes, productivity and affordability.
